A Comparison of Clinical Results for Laparoscopic Cholecystectomy Versus Standard Open Cholecystectomy

Abstract
although many reports of early laparoscopic cholecystectomy were excellent, many surgeons want to know the surgical results of laparoscopic cholecystectomy are enough good to perform comparing with the standard open cholecystectomy.
We compared the diffenence of underwent laparoscopic cholecystecomy(L.C) wit that of standard open cholecystectomy(O.C) to investigated the difference on the sex and age, associated disease, number of previous abdominal operation, duration of
operation,
duration of postoperative hospitalization. pathological diagnosis, postoperative complications and postoperative parenteral narcotic requirement in 600 patients with symptomatic gallstone disease who were treated at the Department of Surgery,
Yong
San
Hospital, Chung-Ang University from September 1990 to December 1991.
There 600 cases were divided into two groups according to the mode of treatment: 300 patients in laparoscopic cholecystectomy group and 300 patients in open cholecystectomy group.
@ES The results were as follows:
@EN 1) The ratio of male to female was 1 : 1.5 in both group. Prevalent age 6th decades for L.C group and 5th decades for O.C group.
2) The associated diseases were 85 cases928.3%) in L.C group and 74 cases(24.6%) in O.C group.
3) The number of previous abdominal operation were 76 patients(25.3%) in L.C group and 55 patients(18.3%) in O.C group. But L.C group were not includeded upper abdominal surgery in past
medical history.
4) The mean operation time were 42.5¡¾19.1 min in L.C group and 49.8¡¾22.6 min in O.C group (P<0.01).
5) The mean of hospitalization after operation was shorter in L.C group(Mean: 5.5¡¾2.3 days) tan O.C group(11.9¡¾2.1 days)(P<0.01).
6) On pathologic finding, no significant difference was noted in both group.
7) No significant difference was noted in postoperative complications of 11 cases in L.C group versus 15 cases in O.C group.
8) In narcotic requirements, 163 cases in operation day, 164 cases in postoperative first day, 96 cases in postoperative 2nd day in L.C group and 236, 168 in O.C group in respectively(P<0.01).
